Ages & Stages
The first five years of life are very important to your child’s
development as this sets the stage for success in school and life. This
is the best time for your child to learn through play and experiences.
The Family Connections Parent Link Centre is pleased to offer “Ages and
Stages” to assist parents and caregivers in following their children’s
growth and development. “Ages & Stages” Questionnaires are completed by
parents and are designed for monthly intervals:
2,4,6,8,9,10,12,14,16,18,20,22,24,27,30,33,36,42,48,54,and 60 months.
Questionnaires are available to parents in three different ways:
- They can be picked up at the Parent Link Centre
- By request, fax, mail or email
- By appointment
Then What???
Parents can complete the questionnaire at home or with a staff (by
appointment) once completed please return or mail your questionnaire
back to the parent link. A staff will summarize the scale and then
discuss with parents or caregiver. Your families’ information remains
confidential at all times!!!!
